How To Get Started

Call for an appointment. Our staff will get you scheduled promptly with one of our physical therapists. You can print off and fill out the patient registration form online and bring it with you to your first visit, or you can arrive early and fill it out in the office. Depending on your insurance plan, a doctor’s referral may be required prior to beginning treatment. We ask that you bring your most recent insurance cards, doctor referral, and any related information that may assist us in your treatment.
